We ask the Government to add the NHS to the list of organisations notified of deaths by the Tell Us Once service. We also want them to require a review of and improvements to all NHS services' communications to ensure communications with bereaved families are coordinated and informed.

This must happen to ensure bereaved families are not sent appointments and treatment information for family members who have already passed away.
